At Gateway Wellness, we provide comprehensive, person-centered mental health and rehabilitative services to help adults live more independently, confidently, and with dignity. Our services are designed for individuals 18 years and older who are experiencing the challenges of serious and persistent mental illness. We strive to make our care welcoming, supportive, and accessible to everyone.

Adult Rehabilitative Mental Health Services (ARMHS)
- Developing coping skills to reduce symptoms of depression, anxiety, and PTSD
- Building routines that support mental and physical well-being
- Strengthening communication and social skills to reduce isolation
- Supporting clients in achieving greater independence in daily life
Basic Living and Social Skills (BLSS)
- Community resource use and integration
- Crisis assistance and relapse prevention
- Budgeting, shopping, and money management
- Cooking, nutrition, and healthy lifestyle practices
- Transportation skills
- Medication monitoring and organization
- Mental health symptom management


Certified Peer Specialist (CPS) Services
- Guidance through shared experiences of recovery
- Support in overcoming stigma, isolation, and personal loss
- Assistance with re-establishing life after hospitalization or crisis
Community Intervention
- Reducing barriers to housing, work, or social participation
- Minimizing risk of hospitalization or restrictive placement
- Working with employers, landlords, institutions, or families to create supportive environments


Functional Assessments
- Review of how symptoms impact daily roles and responsibilities
- Identification of strengths, abilities, and natural supports
- Prioritization of goals for recovery
- Coordination with diagnostic assessments and LOCUS (Level of Care Utilization System)
- Development of an interpretive summary to bridge assessment with service planning
Individual Treatment Plans (ITP)
- Documents goals, strategies, and responsibilities for recovery
- Reflects the client’s vision of recovery and treatment priorities
- Incorporates input from family or supports when appropriate
- Aligns with cultural and language needs
- Ensures clarity, relevance, and active client participation
